Our team work closely with the community neurorehabilitation team and provide the following services.

RAPID RESPONSE TEAM (RRT)
As one of the largest private community home care companies in North London, Responsive Care is here to help – through every stage of your recovery.
We will take over your care plan as soon as you are medically fit to discharge home from the hospital. We have developed the Rapid Response Team (RRT). We liaise with your hospital to identify your needs and coordinate your care within two working days.
Our caring nurses and healthcare assistants have extensive experience treating a person with a brain and spinal cord injuries and other complex neurological conditions.
We will develop a comprehensive discharge plan, including what services and supports you will require in your ongoing recovery. We introduce your key working before your discharge from the hospital, and we will prepare your report for the funding approvals from the local authorities.

PALLIATIVE CARE SERVICES
